Life Certificate

There is no need to take an appointment. Applicants can visit the Embassy/Consulate on any working day. For submission of documents : 9:15 am - 11:15 am. Delivery of documents : 11:30 am - 13:00 pm. The service will be on first come first serve basis. Before, approaching the Consular Office to seek required service, applicant must be ready with complete application form and other required documents, as mentioned in the Embassy website. This is important to render better and quick service to the applicants. Following is the Consular jurisdiction of the Consulate: Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont

The Consulate reserves the right to reject attestation of documents, the contents of which are objectionable or contrary to the Rules.

Note: Consular Services are not provided to those applicants having Asylee or Refugee visa status.

Note: In addition to pension beneficiaries, there is a regular inflow of applicants (both Indian and US passport holders) who want Life Certificate, presumably for reasons other than pension benefits. In such cases the Consulate attests the Affidavit and charges $ 27 ( $ 25 + $2 ).In such cases the applicant has to produce his own form for any other purpose other than pension.

Please Note : Total fee mentioned against each service include fee of US$ 2/- (two) towards Indian Community Welfare Fund (ICWF). This is only onetime fee for first copy of attestation/first copy of certificate, for subsequent certificate/attestation (if required) no ICWF fee will be charged. 

How to Apply (In Person):

1.

Download the Miscellaneous Application form (Form F) and fill it up carefully and affix his/her photograph on the same.

2.

Applicant who is seeking LIFE CERTIFICATE needs to appear in person at the Embassy/Consulate.

3.

Original and a photocopy of the Life Certificate, which has been provided by the Pension Authorities in India. In the absence of this document, Consulate will issue Life Certificate as per its own format. Only one copy of such certificate will be provided to the applicant.

4.

Applicant’s current valid passport in original and its photocopy. Those having Indian passport may provide original and photocopies of the first and last page of the passport.

5.

Original and photocopy of the documents showing valid US visa status.

6.

Original and photocopy of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.

Note: Bank/credit card/mobile phone statements are not accepted as residence proof. 

7.

Carry all original supporting documents for presentation to the Consular Officer. Photocopies of these documents may be attached with the application.  Ensure that photocopies of these documents are legible.

8.

Fee:  Gratis (no fee for one copy) Only US$ 2/- towards ICWF Payment to be made (cash only)

Note : For subsequent copy (if needed), US$ 25/- for each copy has to be paid by applicant (in cash) 

9.

Processing Time: Application will be processed and handed over to applicant on the same day.

Applying by Post:

1.

Download theMiscellaneous Application form (Form F) and fill it up carefully and affix his/her photograph on the same.

2.

For postal applications, a recent (no later than 10 days) certificate from a registered medical practitioner, certifying that applicant (with name) has been examined (with date) by the medical practitioner

3.

Original and a photocopy of the Life Certificate, which has been provided by the Pension Authorities in India duly notarized in the US.

4.

Applicant’s current valid passport in original and its photocopy. Those having Indian passport may provide original and photocopies of the first five pages and last two pages of the passport.

5.

Self attested copy of the documents showing valid US visa status.

6.

Self attested copy (legible) of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.

Note: Bank/credit card/mobile phone statements are not accepted as residence proof.

Fee may be paid by Money order/Cashiers’ Cheque in favor of Consulate General of India, New York

PERSONAL CHEQUES, CREDIT CARDS, DEBIT CARDS OR OTHER BANKING INSTRUMENTS ARE NOT ACCEPTED.

8.

Processing Time: Applications received by postal mail will be processed and dispatched within a maximum 10 business days excluding the time taken in mail.

9.

Please send a track able Pre-paid return mailing envelope to Consulate at Consular section, Consulate General of India, 3 East 64th Street
(Between 5th and Madison Avenues), New York, NY 10065 (this is must).
