Affidavit for Childs Passport to be issued in India

There is no need to take an appointment. Applicants can visit the Embassy/Consulate on any working day. For submission of documents : 9:15 am - 12:15 pm. Delivery of documents : 4:00 pm - 5:00 pm. The service will be on first come first serve basis. Before, approaching the Consular Office to seek required service, applicant must be ready with complete application form and other required documents, as mentioned in the Embassy website. This is important to render better and quick service to the applicants.

Please apply strictly as per your jurisdiction for which you have verifiable proof of address. Following is the Consular jurisdiction of the Consulates: Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont

The Consulate reserves the right to reject attestation of documents, the contents of which are objectionable or contrary to the Rules.

Note: Consular Services are not provided to those applicants having Asylee or Refugee visa status.

 

How to Apply (in Person):

1.

Download the  and fill it up carefully. Each parent is required to fill up separate Miscellaneous Application form (Form F) and affix his/her photograph on the same.

2.

A copy of the affidavit(Form D) (pdf file) in the prescribed proforma has to be submitted in duplicate.

3.

Applicant’s current valid passport in original and its photocopy. Those having Indian passport may provide original and photocopies of the first page, last page and Visa page of the passport.

4.

Original and photocopy of the document showing valid US Visa Status.

5.

Original and photocopy of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.

Note: Credit card/mobile phone statements are not accepted as residence proof. 

5.

Carry all original supporting documents for presentation to the Consular Officer. Photocopies of these documents may be attached with the application.  Ensure that photocopies of these documents are legible.

6.

Fee:  A non-refundable fee of US$10 (ten) + US$2 (two) towards Indian Community Welfare Fee (ICWF) per attestation. The fee may be paid through cash only. .

PERSONAL CHEQUES, CREDIT CARDS, DEBIT CARDS OR OTHER BANKING INSTRUMENTS ARE NOT ACCEPTED.

7.

Processing Time: Application will be processed and handed over to applicant on the same day.

 

How to Apply Through Postal Mail:

1.

Download the Miscellaneous Application form(Form F) and fill it up carefully. Each parent is required to fill up separate Miscellaneous Application form(Form F) and affix his/her photograph on the same.

2.

A copy of the affidavit(Form D) (pdf file) in the prescribed proforma has to be submitted in duplicate. Applicants applying through postal mail may send notarized copy of the affidavit.

Applicant’s current valid passport in original and its photocopy. Those having Indian passport may provide origi9nal and photocopies of the first five pages and last two pages of the passport.

3.

Self attested copy of the document showing valid US Visa Status.

4.

Self attested copy of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.

5.

Fee:  A non-refundable fee of US$10 (ten) + US$2 (two) towards Indian Community Welfare Fee (ICWF) per attestation. The fee may be paid money order/cashiers’ cheque in favor of Consulate General of India, New York

PERSONAL CHEQUES, CREDIT CARDS, DEBIT CARDS OR OTHER BANKING INSTRUMENTS ARE NOT ACCEPTED.

6.

Please send a track able pre-paid return mailing envelope to the Consulate at " 3, East 64 Street, New York 10065 ( between V and Madison Avenue)

7.

Processing Time: Application will be processed within Five (5) working days from the day of receipt of documents by postal mail.
