Birth Certificate or Non-availability of Birth Certificate

There is no need to take an appointment. Applicants can visit the Embassy/Consulate on any working day.For submission of documents : 9:15 am - 11:15 am. Delivery of documents : 11:30 am - 13:00 pm. The service will be on first come first serve basis. Before, approaching the Consular Office to seek required service, applicant must be ready with complete application form and other required documents, as mentioned in the Consulate website. This is important to render better and quick service to the applicants.

Following is the Consular jurisdiction of the Consulate: Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont

The Consulate reserves the right to reject attestation of documents, the contents of which are objectionable or contrary to the Rules.

Note: Consular Services are not provided to those applicants having Asylee or Refugee visa status.

The Consulate does not attest any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, diplomas, marriage certificate and educational certificates issued by authorities in India) are required to be presented to authorities in USA, these should be apostilled in India. Birth Certificate is issued on the basis of entries in Indian passport. Therefore, it can only be issued if an Indian passport is presented in original at the counter or mail.

Non availability of birth certificate can be issued to those Indian citizens who present their Indian passport in original and who were born in India before 2nd June 1969.

Please Note : Total fee mentioned against each service include fee of US$ 2/- (two) towards Indian Community Welfare Fund (ICWF). This is only onetime fee for first copy of attestation/first copy of certificate, for subsequent certificate/attestation (if required) no ICWF fee will be charged. 

How to Apply (in person):

1.

Download the Miscellaneous Application form(Form F)and fill it up carefully. Each applicant is required to fill up separate Miscellaneous Application form(Form F)and affix his/her photograph on the same.

2.

Applicant’s current valid passport in original and its photocopy.

Applicant who is a non-Indian passport holder may provide his/her old Indian passport in original. Indian passport holders may provide original and photocopies of the first and last page of the passport.

3.

Original and photocopy of the documents showing valid US visa status.

4.

Original and photocopy of any of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.

Credit card/mobile phone statements are not accepted as residence proof. 

5.

Carry all original supporting documents for presentation to the Consular Officer. Photocopies of these documents may be attached with the application.  Ensure that photocopies of these documents are legible.

6.

Fee:  A non-refundable fee of US$27.00 per copy. The fee may to be paid in cash only.

7.

Processing Time: Same day if applied in person


How to Apply by Postal Mail:

1

Download the Miscellaneous Application form(Form F) and fill it up carefully. Each applicant is required to fill up separate Miscellaneous Application form(Form F) and affix his/her photograph on the same.

2.

Applicant’s current valid passport in original and its photocopy.

Applicant who is a non-Indian passport holder may provide his/her old Indian passport in original. Indian passport holders may provide original and photocopies of the first page, last page and the Visa page of the passport.

3.

Self attested copy of the documents showing valid US visa status.

4.

Self attested copy of the following documents as proof of residence: US Driving License, Utility Bill, Lease Agreement, State Identity Card, and Mortgage Deed.

Credit card/mobile phone statements are not accepted as residence proof

5.

Fee: A non-refundable fee of US$ 27/- per copy. For subsequent documents (if required) , US$ 25/- per copy has to be paid by applicant.

The fee may be paid through money order/cashiers’ chequein favor of Consulate General of India, New York.

PERSONAL CHEQUES, CREDIT CARDS, DEBIT CARDS OR OTHER BANKING INSTRUMENTS ARE NOT ACCEPTED.

6.

Please send a track able Pre-paid return mailing envelope to the Consulate.at Consulate General of India, Consular Wing, 3 East, 64th Street, New York, NY 10065 (this is Must).

7.

Processing time: Maximum of Ten working days from the date of receipt of postal mail at the Consulate. This is subject to submission of all requisite documents by applicant(s).
